Abstract

Direct oral anticoagulants have become first-line agents in anticoagulation therapy in recent years; however, their reversal may be challenging in emergency situations, such as massive bleeding or urgent surgery. Conventional coagulation tests are not suitable for accurately assessing anticoagulant activity, making specific monitoring methods (e.g., anti-FXa, viscoelastic tests) essential. Among antidotes, idarucizumab is effective for rapidly reversing the effects of dabigatran, while andexanet alfa neutralizes apixaban and rivaroxaban, though its availability and thrombosis risk limit its use. Alternative strategies include prothrombin complex concentrate or extracorporeal techniques. Determining anticoagulant plasma concentrations before urgent interventions aids clinical decision-making, but in critical cases, hemostatic therapy should commence without awaiting lab results. Following bleeding management, timely reinitiation of anticoagulation is crucial to mitigate thrombotic risk. Orv Hetil. 2025; 166(27): 1043–1052. A direkt orális antikoagulánsok az elmúlt években a véralvadásgátló kezelés első vonalbeli szereivé váltak, azonban sürgősségi helyzetekben – például masszív vérzés, akut műtét – kihívást jelenthet hatásuk felfüggesztése. A hagyományos véralvadási vizsgálatok nem nyújtanak megbízható információt e szerek hatásának pontos felmérésére, ezért specifikus monitorozási módszerek (például anti-FXa-teszt, viszkoelasztikus tesztek) alkalmazása javasolt. Orv Hetil. 2025; 166(27): 1043–1052.

Zusammenfassung

Direct oral anticoagulants have become first-line agents in anticoagulation therapy in recent years; however, their reversal may be challenging in emergency situations, such as massive bleeding or urgent surgery.

Warum dies für die Hirudotherapie relevant ist

This review provides practical guidance on emergency management of DOAC-treated patients, discussing reversal strategies including idarucizumab for dabigatran, andexanet alfa for apixaban and rivaroxaban, prothrombin complex concentrate as an alternative, and the importance of timely anticoagulation reinitiation after bleeding control. The abstract does not mention leeches, hirudotherapy, hirudin, or any leech-derived compounds at any point. Its focus is entirely on clinical emergency management of synthetic oral anticoagulant-related bleeding and urgent surgical scenarios. There is no defensible connection to ASH's domain of hirudotherapy or the leech secretome based on this abstract.
