Abstract
Medical leeches (Hirudo medicinalis) in plastic and reconstructive surgery are often used for the treatment of vascular failure after microvascular surgery. Leeches are a reservoir for bacteria of the Aeromonas group that help digesting the blood meal. In some cases these bacteria are able to cause severe wound infections that can lead to loss of tissue transplants. We report about a patient with a common microvascular forearm flap after resection of an oral squamous cell carcinoma which got infected by Aeromonas spp. after treatment with medical leeches. Most of these species are resistant for common antibiotic treatment after surgery. This report shows the importance of an early concomitant antibiotic prophylaxis in the treatment of venous congestion with medical leeches.

Dieser Bericht beschreibt einen Patienten, dessen mikrovaskulärer Unterarmlappen, der nach Resektion eines Plattenepithelkarzinoms der Mundhöhle eingesetzt wurde, sich nach der Behandlung mit dem medizinischen Blutegel (Hirudo medicinalis) wegen venöser Stauung mit Aeromonas-Spezies infizierte; die Autoren merken an, dass Aeromonas — der Blutverdauungs-Symbiont des Blutegels — eine schwere Wundinfektion verursachte, die häufig gegenüber gängigen postoperativen Antibiotika resistent ist, und betonen eine frühe begleitende Antibiotikaprophylaxe, wenn Blutegel bei venöser Stauung eingesetzt werden. Für ASH ist dies ein wichtiger Sicherheitsdatenpunkt: Er dokumentiert die reale, transplantatgefährdende Kehrseite der bekanntesten Komplikation der Blutegeltherapie und stützt unmittelbar eine protokollierte Antibiotikaprophylaxe. Als einzelner Fallbericht weist er das niedrigste Niveau klinischer Evidenz auf — ein Patient, keine Vergleichsgruppe —, sodass er ein bekanntes Risiko und die Begründung für eine Prophylaxe veranschaulicht und untermauert, anstatt Infektionsraten zu quantifizieren oder ein Prophylaxeschema zu beweisen.
