Encyclopedist with a love of detail: Aulus Cornelius Celsus and his dermatological treatments

Abstract
BACKGROUND: The Roman author Aulus Cornelius Celsus wrote an eight volume encyclopedia of medicine. His remarks on terminology are considered paradigmatic in all fields of medicine. The dermatological pathology of Celsus has so far been analyzed mainly in summary descriptions of the medical knowledge of antiquity. A modern text-based interpretation of the contributions of the author to the diagnosis and treatment of skin diseases is missing. OBJECTIVES: What kind of diagnostic and therapeutic expertise do the dermatological reports of Celsus convey? MATERIALS AND METHODS: The medical encyclopedia of Celsus has been examined for anamnestic, diagnostic, therapeutic and prognostic statements on diseases of the skin. RESULTS: The work of Celsus contains 31 chapters and notes in which diseases of the skin are described. By far the most are found in the fifth book which is dedicated to pharmacotherapy. The majority of the texts are divided into information on anamnesis and location, the results of the physical examination and the description of the changing aspect of the lesions under treatment. Ulcers, gangrene, erysipelas, boils and carbuncles, papules, pustules, impetigo and scabies are represented in particular detail. Elephantiasis, the so-called sacred fire and alopecia occupy a distinct position. In addition to the treatment with topically applicable remedies, the author discusses surgical measures and the use of cupping glasses. CONCLUSION: Reading the dermatological reports and commentaries of the author shows that he saw in the efflorescences both isolated diseases and signs of generalized suffering. A perfect example of this evaluation is inflammatory redness. On the one hand, many of the names used by Celsus for skin diseases are firmly embodied in current terminology; on the other hand, a considerable number of appellations can not be found in modern nomenclature, despite their well-defined importance for differential diagnosis. The use of cupping heads and bloodletting was widespread within the treatment of dermatoses, because the so-called bad juices were held responsible for the development of the majority of efflorescences due to the lack of differentiated etiological knowledge of the physicians. ZUSAMMENFASSUNG: HINTERGRUND: Der römische Fachautor Aulus Cornelius Celsus hat eine Enzyklopädie der Medizin in 8 Büchern verfasst. Seine Ausführungen zur Terminologie gelten für sämtliche Disziplinen der Heilkunde als beispielhaft. Die dermatologische Krankheitslehre des Celsus ist bisher überwiegend in zusammenfassenden Beschreibungen der medizinischen Kenntnisse der Antike gewürdigt worden. Eine moderne textbasierte Darstellung der Beiträge des Celsus zur Erkennung und Behandlung der Erkrankungen der Haut fehlt. ZIEL DER ARBEIT: Welche diagnostische und therapeutische Fachkompetenz vermitteln die dermatologischen Ausführungen des A. C. Celsus? MATERIAL UND METHODEN: Die medizinische Enzyklopädie des Celsus wurde auf anamnestische, diagnostische, therapeutische und prognostische Aussagen zu Erkrankungen der Haut geprüft. ERGEBNISSE: Das Werk des Celsus enthält 31 Kapitel und Notizen, in denen Erkrankungen der Haut dargestellt sind. Mit Abstand die meisten (n = 21) finden sich in dem der Pharmakotherapie gewidmeten fünften Buch. Die Mehrzahl der Texte gliedert sich in Angaben zu Anamnese und Lokalisation, den Ergebnissen der körperlichen Untersuchung und der Schilderung des sich unter der Behandlung wandelnden Aspekts der Effloreszenzen. Besonders detailreich werden Ulzera, Gángraina, Erysípelas, Furunkel und Karbunkel, Papeln, Pusteln, Impetigo und Scabies beschrieben. Eine Sonderstellung nehmen Elephantiasis, das sog. Heilige Fieber und die Alopezie ein. Über die Behandlung mit Externa hinaus bespricht der Verfasser chirurgische Maßnahmen und den Einsatz von Schröpfköpfen. DISKUSSION: Die Lektüre der dermatologischen Berichte und Kommentare des Celsus zeigt, dass er in den Effloreszenzen sowohl isolierte Erkrankungen als auch Zeichen generalisierter Leiden sah. Das Paradebeispiel für diese Interpretation ist die entzündliche Rötung. Einerseits sind viele von Celsus verwendete Bezeichnungen für Erkrankungen der Haut in der aktuellen Terminologie fest verankert. Zum anderen ist eine beträchtliche Zahl von Begriffen trotz unverkennbar großer Bedeutung für die Differenzialdiagnose in der modernen Nomenklatur nicht zu finden. Auf den Einsatz von Schröpfköpfen und den Aderlass wurde auch bei der Behandlung von Dermatosen nicht verzichtet, weil man aus Mangel an differenzierten ätiologischen Kenntnissen die sog. schlechten Säfte für die Entstehung der meisten Effloreszenzen verantwortlich machte.

Este artículo histórico analiza las enseñanzas dermatológicas del enciclopedista romano Aulus Cornelius Celsus, examinando su enciclopedia médica de ocho volúmenes en busca de enunciados diagnósticos, terapéuticos y pronósticos sobre enfermedades cutáneas, entre ellas úlceras, gangrena, erisipela, forúnculos, ántrax, pápulas, pústulas, impétigo y escabiosis. El resumen señala que Celso discutió medidas quirúrgicas, remedios tópicos y el uso de ventosas (Schröpfköpfe) y sangrías (Aderlass) en el tratamiento de las dermatosis, reflejando la teoría humoral de los «malos humores».
