Management of the partial avulsions of the auricle

Abstract
Injuries of the auricle can range from simple lacerations to complete avulsions. Many techniques of ear replantation have been described in the literature in addition to the type and extent of the involved auricular structures. A direct reattachment of the amputated pinna without microsurgery is rarely successful due to necrosis of the avulsed fragment. Whereas, reconstructions with pocket methods and their variations might lead to better results. In this article we would like to discuss some of these approaches and demonstrate a two-stage reconstruction technique for subtotal avulsion of the auricle. Ohrmuschelverletzungen zählen zu den selteneren Traumata im Kopf-Hals-Bereich. Aufgrund der komplexen Anatomie des Knorpels und der begrenzten Blutversorgung bedürfen sie einer raschen und strukturierten Behandlung. Für die Behandlung sowohl von leichten Verletzungen als auch von (sub-)totalen Abrissen der Ohrmuschel stehen unterschiedliche Therapieoptionen zur Verfügung. Eine direkte Readaptation einer (sub-)total abgerissenen Ohrmuschel ohne mikrovaskulare Anastomose ist aufgrund des hohen Risikos von Haut- und Knorpelnekrosen nur selten erfolgreich. Mehrzeitige Rekonstruktionen mit Pocket-Methoden sind dagegen gut etabliert. Die vorliegende Arbeit beschreibt einige dieser Ansätze und demonstriert eine 2-stufige OP-Technik eines Ohrmuschelteilabrisses aus dem eigenen Patientengut.

Este artículo analiza los enfoques de manejo para las avulsiones parciales y subtotales del pabellón auricular (oído externo), comparando la reattachment directa, los métodos de bolsillo y las técnicas de reconstrucción por etapas. Los autores presentan una técnica de reconstrucción en dos etapas para la avulsión auricular subtotal a partir de su propia población de pacientes, señalando que la reattachment directa sin anastomosis microvascular rara vez es exitosa debido al alto riesgo de necrosis cutánea y del cartílago. El resumen no contiene ninguna mención de sanguijuelas, hirudoterapia, hirudina, ni de ningún agente terapéutico derivado de sanguijuelas.
