The Current Landscape

The United States does not yet have a single, federally recognized certification standard for hirudotherapy practitioners. The American Society of Hirudotherapy is working to establish comprehensive national standards that ensure consistent quality of care across the country. In the meantime, practitioners should pursue the best available training and operate within their existing professional licensure.

Core Competencies for Practitioners

A qualified hirudotherapy practitioner should demonstrate competency in these key areas:

Leech Biology & Pharmacology

Understanding the biology of medicinal leeches, their salivary compounds, and mechanisms of action. This includes knowledge of hirudin, bdellins, eglins, destabilase, and other bioactive molecules.

Clinical Assessment & Patient Selection

Ability to evaluate patients for appropriate use of hirudotherapy, identify contraindications, obtain informed consent, and develop individualized treatment plans. Understanding which conditions are FDA-cleared, evidence-supported, or investigational.

Application Techniques

Hands-on skills for proper leech application: site preparation, leech placement, monitoring during treatment, safe removal, and post-treatment wound care. Includes knowledge of leech behavior and troubleshooting.

Safety & Infection Control

Strict protocols for infection prevention including the use of FDA-cleared leeches only, single-use policy, Aeromonas prophylaxis, wound monitoring, and proper biohazard disposal. Understanding of complications and their management.

Regulatory & Legal Framework

Understanding of FDA device regulation (510(k) clearance, pre-amendment status), scope of practice within your professional license, informed consent requirements, documentation standards, and liability considerations.

Evidence-Based Practice

Ability to critically evaluate clinical research, stay current with the latest published evidence, and apply evidence-based decision-making to patient care. Understanding of different levels of clinical evidence.

Who Can Practice Hirudotherapy?

In the United States, hirudotherapy should be practiced by licensed healthcare professionals operating within their scope of practice. The following professionals are well-positioned to add hirudotherapy to their services:
