Training & Certification
Your pathway to becoming a qualified hirudotherapy practitioner in the United States
Interest in hirudotherapy is growing across the United States. As an FDA-cleared medical device used at major academic medical centers, there is increasing demand for properly trained practitioners who can deliver this treatment safely and effectively.
Whether youโre a physician, nurse, physical therapist, or other healthcare professional, this page outlines the knowledge, skills, and training you need to practice hirudotherapy with confidence.
Why Certification Matters
Patient Safety
Proper training ensures you understand infection control, contraindications, and emergency protocols. Your patients deserve a practitioner who follows the highest safety standards.
Professional Credibility
Certification demonstrates your commitment to evidence-based practice. It builds trust with patients, colleagues, and referring physicians.
Growing Demand
As clinical evidence grows and patient interest increases, qualified hirudotherapy practitioners are in higher demand. Certification positions you at the forefront of integrative medicine.
The Current Landscape
Core Competencies for Practitioners
A qualified hirudotherapy practitioner should demonstrate competency in these key areas:
Leech Biology & Pharmacology
Understanding the biology of medicinal leeches, their salivary compounds, and mechanisms of action. This includes knowledge of hirudin, bdellins, eglins, destabilase, and other bioactive molecules.
Clinical Assessment & Patient Selection
Ability to evaluate patients for appropriate use of hirudotherapy, identify contraindications, obtain informed consent, and develop individualized treatment plans. Understanding which conditions are FDA-cleared, evidence-supported, or investigational.
Application Techniques
Hands-on skills for proper leech application: site preparation, leech placement, monitoring during treatment, safe removal, and post-treatment wound care. Includes knowledge of leech behavior and troubleshooting.
Safety & Infection Control
Strict protocols for infection prevention including the use of FDA-cleared leeches only, single-use policy, Aeromonas prophylaxis, wound monitoring, and proper biohazard disposal. Understanding of complications and their management.
Regulatory & Legal Framework
Understanding of FDA device regulation (510(k) clearance, pre-amendment status), scope of practice within your professional license, informed consent requirements, documentation standards, and liability considerations.
Evidence-Based Practice
Ability to critically evaluate clinical research, stay current with the latest published evidence, and apply evidence-based decision-making to patient care. Understanding of different levels of clinical evidence.
Who Can Practice Hirudotherapy?
In the United States, hirudotherapy should be practiced by licensed healthcare professionals operating within their scope of practice. The following professionals are well-positioned to add hirudotherapy to their services:
๐ฅ Hospital & Clinical Settings
- โ Physicians (MD, DO) โ Plastic surgeons, vascular surgeons, orthopedic surgeons, and integrative medicine physicians
- โ Registered Nurses (RN) โ Under physician supervision, particularly in post-surgical care
- โ Physician Assistants (PA) โ Within their collaborative practice agreements
- โ Nurse Practitioners (NP) โ Within their scope of practice
๐ฟ Integrative & Outpatient Settings
- โ Naturopathic Doctors (ND) โ In states where NDs have prescriptive authority
- โ Licensed Vocational Nurses (LVN) โ Under appropriate medical supervision
- โ Licensed Acupuncturists (LAc) โ Where state law permits
- โ Physical Therapists (PT/DPT) โ For musculoskeletal applications within scope
Important: Know Your Scope
Training Resources
Several organizations offer hirudotherapy training in the United States. We recommend programs that include both didactic (classroom) and hands-on clinical components.
What to Look for in a Training Program
BTER Foundation Workshops
The BioTherapeutics, Education & Research Foundation offers workshops on the principles and practice of biotherapy, including hirudotherapy. Programs include hands-on training with live or mock patients.
Location: Southern California & various U.S. locations
ASH Educational Programs

National Certification โ In Development
The American Society of Hirudotherapy is developing a national certification program for hirudotherapy practitioners. This program is currently in the planning and curriculum development phase. No certification is currently issued or available. When launched, the program will aim to set evidence-based standards for training, clinical competency, and patient safety.
The planned program is expected to include:
