Building a Practice

Equipment, suppliers, facility setup, and patient communication

Last Updated: March 1, 2026Reviewed by: Andrei Dokukin, MD

Establishing a hirudotherapy practice requires careful attention to regulatory compliance, equipment procurement, leech sourcing, facility requirements, and patient communication. The estimated startup cost for adding leech therapy to an existing clinical facility is ,000-,000, excluding leeches and facility buildout.

FDA-Cleared Leech Suppliers

Ricarimpex SAS

Eysines, France

  • 510(k): K040187 (2004)
  • First FDA clearance for medicinal leeches
  • Primary US market supplier (~80,000+ leeches/year)
  • 150+ years of leech breeding experience

Biopharm Leeches Ltd

Hendy, Wales, UK

  • 510(k): K132958 (2014)
  • ~20,000 leeches/year to US market
  • Educational resources and technical support
  • Established secondary source

Carolina Biological Supply Co.

Burlington, NC

  • 510(k): K140907 (2015)
  • Primarily scientific/educational supplier
  • Also offers FDA-cleared clinical leeches
  • US-based distribution

Practitioners must never source leeches from pet stores, bait shops, wild-caught populations, or non-FDA-cleared suppliers. Using non-cleared leeches creates product liability exposure and violates medical device regulations.

Facility Requirements

Treatment Room

  • Private, well-lit clinical room
  • Examination table or reclining chair
  • Smooth, easily cleaned surfaces
  • Running water and handwashing
  • Biohazard waste container
  • Emergency equipment within reach

Storage Area

  • Temperature-controlled (50-68°F / 10-20°C)
  • Dedicated mini-refrigerator or climate-controlled room
  • Inaccessible to unauthorized personnel
  • Low ambient light (photosensitivity)
  • Clearly labeled

Decontamination Area

  • Designated for cleaning instruments
  • Biohazard waste processing
  • Soiled material handling
  • OSHA Bloodborne Pathogen compliant

Patient Communication

Initial Consultation Strategy

  1. Contextualize the therapy: FDA-cleared medical devices used at major academic centers
  2. Explain mechanism in accessible language: Natural compounds that thin blood, reduce inflammation, improve circulation
  3. Use visual aids: Photographs or video clips of treatment in progress
  4. Address emotional responses: Acknowledge that discomfort with leeches is entirely normal

Pre-Session Instructions

  • Avoid lotions, perfumes, topical products on treatment area
  • No alcohol for 24 hours before treatment
  • Eat a normal meal before appointment (vasovagal prevention)
  • Wear loose, comfortable clothing with easy access

During Session

  • Initial bite: comparable to mild pinch (anesthetic takes effect in 1-2 min)
  • Feeding: 20-45 minutes, leech detaches naturally
  • Mild itching may begin during or shortly after

Staffing & Operations

Staffing Considerations

While a solo practitioner can perform hirudotherapy independently, a trained assistant significantly improves safety and efficiency — monitoring multiple leeches, tracking feeding duration and blood loss, assisting with documentation, and monitoring for adverse reactions. All staff with potential blood exposure must receive OSHA bloodborne pathogen training, and the practice must maintain a written Exposure Control Plan updated annually.

Ordering logistics: Ordering logistics: Most suppliers require 48-72 hours advance notice. Leeches ship overnight in insulated containers. Establish accounts with at least one primary and one secondary FDA-cleared supplier. A reasonable initial order is 50-100 leeches, with reorders planned based on treatment volume.

Medicinal leech therapy carries clinically meaningful risks and should be performed only by qualified clinicians under institutionally approved protocols.
